WDTS Training Application

Interested in applying? There are 2 steps required for you to apply for WDTS and both must be done for us to be able to complete your application:

			Step 1 - Fill out Application... All applicants MUST fill out this document and fax or mail to our main office.
			Step 2 - Send us references... 3 References are required for each applicant. This form MUST be filled out by your 3 references and sent to us. Email this form to your references or have them download it from this website.

NOTE: Please remember to include a photograph with all applications!
